During your visit, if you require a prescription, the Raytown Schools Quality Care Clinic providers may:
Dispense pre-packaged medication onsite, if available in the Raytown Schools Quality Care Clinic
Send your prescription electronically to be mailed directly to your home through the Home Delivery Pharmacy Program, if it is clinically appropriate and the preventive/chronic medication is on the formulary
Send it to the local pharmacy of your choice if the medication is not available onsite or through the Home Delivery Pharmacy Program

After completing a thorough assessment, the providers can write a prescription during your visit.
The Raytown Schools Quality Care Clinic will be able to dispense certain medications at the point of care. Medications must be prescribed by one of the Clinic providers during a visit to the Raytown Schools Quality Care Clinic. The Clinic cannot fill prescriptions from outside providers like a pharmacy, such as Walgreens, can fill. The Raytown Schools Quality Care Clinic formulary consists of the top utilized medications and the medications used most commonly for diagnoses in a primary care office. No narcotics or controlled substances will be stocked in the Raytown Schools Quality Care Clinic.
If the medication the Physician has prescribed you is available in the Raytown Schools Quality Care Clinic dispensary, you can conveniently obtain your medication during your visit! If the medication the Physician has prescribed you is not available in the clinic, you can still fill your prescription at your preferred pharmacy or through mail order, whichever you prefer.
Please Note: The Raytown Schools Quality Care Clinic is staffed with a Physician and Nurse Practitioner. In the state of Missouri a Nurse Practitioner is NOT able to dispense medication. With this, based on the provider’s assessment, if you are in need of a prescription medication, the Nurse Practitioner IS able to write a prescription, either via paper or electronically so that you can fill at your preferred pharmacy. If you are needing to obtain medication refill for a dispensed medication from the Clinic, you must do so through an appointment with the Physician.
Dispensing laws require that when a physician dispenses medication that they must see and evaluate the patient. This is different from the requirements of pharmacists in a traditional retail pharmacy. For this reason, an appointment is required to get a refill from the Raytown Schools Quality Care Clinic.
The formulary is subject to change based on utilization, need and cost. To inquire if your medication is kept in stock, please contact Premise Health Member Services, or speak with the Clinic staff during your next visit.
Through the Home Delivery Pharmacy program, patients who are prescribed with certain preventive/chronic medications by a provider of the Raytown Schools Quality Care Clinic, may be eligible to receive them directly to their home at NO COST!

Step 1: Discuss with a Raytown Schools Quality Care Clinic Provider
If you are currently taking a medication that is available through the Home Delivery Pharmacy Program (click here to view the list), schedule an appointment with a provider at the Raytown Schools Quality Care Clinic. Bring your medication list (including those prescribed by community providers; the Raytown Schools Quality Care Clinic will have the information on any medications they have prescribed for you).
Step 2: Provider Evaluation and Prescription
During your appointment, the Raytown Schools Quality Care Clinic provider will review your medications to ensure they align with your clinical needs. They will collaborate with you and any external providers if necessary.
If the medication is appropriate based on their clinic consultation, the Raytown Schools Quality Care Clinic provider will prescribe it, thereby enrolling you in the Home Delivery Pharmacy Program.
Note: Only the Raytown Schools Quality Care Clinic providers have access to this program.
Step 3: Medication Delivery
Once enrolled in the Home Delivery Pharmacy Program, your prescribed medication will be delivered directly to your home.
Medications delivered to your home through the Home Delivery Pharmacy Program will be at no cost to you.
Premise Health offers an expanded formulary for preventive/chronic use medications which cover most of the commonly encountered conditions such as: hypertension, high cholesterol, diabetes, cardiac conditions, etc.
The Home Delivery Pharmacy Program also gives the Raytown Schools Quality Care Clinic provider options to offer therapeutic alternatives to the more costly medications that some patients are currently being prescribed. Only medications present on the formulary list are eligible for the Home Delivery Pharmacy Program.
