Employees covered on the District's health plan who earn 100 points through the Wellness Program by 4/30/26 will avoid the monthly surcharge on health plan premium of $35 for the July 1, 2025 plan year.* 

Completing the program saves you $420 per year!

    • • • How to Register with My Premise Health for the First Time: Click here for instructions.

        • How to Schedule a Biometric Screening Appointment at the Clinic : You can schedule an appointment by calling (816) 744-8030, through the portal at mypremisehealth.com or on the app. Please be sure to schedule a ‘Biometric Screening Visit’. 

        • How to eCheck-in Before Appointments : Click here to learn how to eCheck-in with 7 days prior to your biometric  screening and screening follow up appointments. Be sure to complete all eCheck-in steps, including Signed Documents - you must complete the Incentive Program Employee Notice and Authorization. If you do not complete this form, or you elect to opt out, your points will not count towards your incentive.

        • How to Find Your Lab Results : To view your lab results, sign in to My Premise Health and click 'Test Results' on the main dashboard.

        • How to Check Your Compliance Status :  Click here to view the instruction sheet, or follow these steps:

          • • Sign into My Premise Health online or in the app and click ‘Sonic Boom Wellness’ from the home dashboard or the Menu list under 'My Record'. You will automatically be taken to the Rewards page, which will show which activities have been completed and how many points you have earned.

  • Completing the wellness incentive program with an outside provider instead of through the Raytown Schools Quality Care Clinic?

    • • • You will need to print the Remote PCP Form: Biometric Screening Visit and the Remote PCP Form:  Provider Wellness Review or obtain them from the District HR Office.  Note: The linked forms above include the ‘Incentive Program Employee Notice and Authorization’ form that must be signed and returned (only one time during the incentive; If you completed the biometric screening with your outside provider and already signed and return this authorization form, you do not have to re-sign or re-submit this form again). 

        • These forms must be completed by your provider and then hand-delivered by you to the Raytown Schools Quality Care Clinic (during clinic operating hours) no later than the following dates. Please hand them directly to a care team member and do not place them on the counter and leave.  

          • • • 'Remote PCP Form: Biometric Screening Visit' due by 2/28/26

              • 'Remote PCP Form: Provider Wellness Review' due by 4/30/26

        • Please Note: A visit with or labs performed by an outside provider may incur additional charges. These services are no cost at the Raytown Schools Quality Care Clinic.

*Participation in the incentive entails submitting health information to Premise Health. If you would like an alternative way to earn the wellness incentive, or your doctor recommends an alternative program, please contact Chelsea Huggins by email, chelsea.huggins@cbiz.com, by October 1, 2025.

Important to Remember

• • • Don’t wait until the last minute to schedule your biometric screening appointments! We encourage you to begin scheduling appointments as soon as possible - you can complete your biometric screening as early as May 1, 2025. 

    • Fasting is not required if you are completing your biometric screening through the Clinic. You will be asked whether or not you have fasted at the time of your blood draw; be sure to indicate whether have or have not fasted to ensure the most accurate results.

    • In an attempt to ensure the Raytown Schools Quality Care Clinic schedule retains availability for other visits, including same/next day sick visits, please do not schedule your own Provider Wellness Review Appointment for Step 2. If you do not achieve the screening benchmarks, the Raytown Schools Quality Care Clinic care team will work with you directly to schedule your Screening Follow Up Appointment.
